Dr Chen is an experienced board-certified dermatologist with specialty knowledge and fellowship training in surgical dermatology, Mohs micrographic surgery, aesthetic reconstruction, pathology, and skin cancer management. If patients do not follow practice policies and do not take their financial responsibility seriously, Dr Chen will not be able to continue to see patients and may have to close her practice. Adherence to these guidelines will ensure that patients’ care is coordinated smoothly.

At Chen Skin & Cancer Surgery, English is the primary and official language for all forms of communication (ie. spoken, written, website, etc). Please be advised that any translation may be inaccurate. It is the patient's responsibility to review the English version to ensure translation accuracy.
Dr Chen is an American-born Chinese (ABC) and has limited proficiency in conversational Mandarin Chinese. She is not able to read Chinese. Dr Chen completed 4 years of Spanish (middle and high school) and has limited proficiency in conversational and medical Spanish.

Your appointment time is scheduled specifically for you, and you will receive your provider’s full attention during that time. Together, you and your provider will cover your concerns as well as review your care plan and any upcoming medications or lab orders.

SURGERIES & PROCEDURES
SAME-DAY PROCEDURE & CONSULT AVAILABLE
We know your time is precious. Although some patients prefer separate appointments with the consultation visit first and surgery on a separate day, our office understands how difficult it is to take time off from school and work.
At Chen Skin & Cancer Surgery, same day procedure and consultation may be available. If you are interested in having the procedure performed on the same day as the consultation, please be sure to notify Dr Chen and her staff of your preference and send several photos of the area so that adequate time can be blocked for the procedure (exceptions include large size, multiple locations, complex surgery, time permitting, etc).

GETTING READY FOR YOUR PROCEDURE
No need for driver (in most cases) and no need to fast before surgery since patients will not be put to sleep. All procedures are with local numbing just like at the dentist. Please continue all medications. Do not stop the blood thinners. Please remember that all bleeding stops, and consider wearing a dark color outfit just in case. Dress comfortably to allow easy access to the areas that will be operated on. Don't forget to bring something to pass the time, stay warm, and/or snacks if desired.
Although you can go back to work the same day, many patients take it easy for a day or two (ie. no heavy lifting, use your common sense). Follow-up for wound check/ suture removal is typically 1 week (face) and 2 to 3 weeks (off of the face). Plan to be at the office between 1 to 5 hours (will depend on the complexity of the procedure; hopefully it won't take the whole time). Exceptions may apply if multiple lesions, complexity, size, location, medical problems, etc.

PATIENT AND STAFF ONLY IN THE SURGERY ROOM DURING PROCEDURES
Due to safety regulations, only patient and surgery staff can be in the surgery room during procedures. Family and friends will be asked to wait in the reception area during procedures and surgeries. Thank you for your cooperation with regard to our patient safety policies.
PHONE AND VIDEO CONSULTATIONS
When appropriate, phone and video consultations are available for established patients upon request and are billed at the standard rates using standard insurance phone visit codes. While in-person consultation and examination is the gold standard for quality and exceptional care, some medical issues may not require an in-person exam and could be handled with an Evisit/ Telehealth appointment, such as: wound check, medication refill, medical questions, etc. Other medical conditions are better suited for in-office appointment: skin cancer screening, rash/ eczema, and of course, if procedures and surgeries are indicated.
APPOINTMENT REMINDERS
Chen Skin and Cancer Surgery, PA, does not guarantee appointment reminders; it is up to the patient to remember appointments made. As a courtesy, after the office staff upload patient details, the appointment system will text an appointment confirmation and patient instructions (ie. forms, directions, fees, etc) within 1 to 2 business days, as well as, approximately 1 week before the reserved consultation appointment.
ON-TIME PERFORMANCE
This office is different from others in that there is usually no waiting. Because your appointment time is reserved especially for you and never double-booked, plan to arrive early for your appointment time.
Please call if you are running late to prevent unnecessary worry. Repeat and/or excessively tardy patients may be at risk for termination. The full allotment of time for your appointment is necessary for the doctor to adequately assess the personal and medical issues presented at that time. This policy is in effect to assure the highest quality of care and comprehensive treatment for everyone scheduled.

RESCHEDULING & CANCELLATIONS
If your appointment is cancelled on short notice or if you fail to show for your appointment, we are not able to offer that time to another patient. Thus, any cancellation or rescheduling request must be made no later than 48 hours before your appointment so as to allow another patient to take that open appointment time, as well as, avoid Forfeit of Deposit and/or a Missed Appointment Fee, which must be paid before another appointment can be reserved. (Note that if your appointment falls on a Monday, then any rescheduling should happen by Friday of the previous week.)

If you are a new patient, 50% of the office visit fee will be charged, with the rest of the fee charged the day of your appointment. Chen Skin and Cancer Surgery, PA, makes every effort to send you to an in-network lab. Please note, though, that ultimately you are responsible for ensuring that your insurance will pay for the labs at the testing center at which your labs are drawn. There is no on-site lab draw available at this time.
For select tests, Chen Skin and Cancer Surgery, PA, is able to offer laboratory testing at a special negotiated fee outside of the insurance system. If you choose to use this service, payment for laboratory testing is due at the time the labs are ordered. Please notify our office to avail yourself of this option.
Should you choose a lab where Chen Skin and Cancer Surgery, PA, does not have an account, please make sure the lab results are sent directly to us via fax at (888) 381-4541. Please also check with the lab in 1 to 2 weeks to confirm the results have been reported.

LAB RESULTS
Lab results are reviewed in appointments only.
If your lab results are available before your appointment, your provider will send them to you with a brief comment. All lab results will then be reviewed in detail at your appointment. Please do not ask for interpretation by phone or email. Should labs need to be reviewed urgently, an appointment will be set up within 48 hours.
PRESCRIPTIONS
To ensure your safety, Chen Skin and Cancer Surgery, PA, has developed the following policies regarding prescriptions.

If a patient's last consultation and in-office skin examination with Dr Chen is more than 365 days, Dr Chen's standard practice requires the patient return to the office for assessment and examination to reestablish care before a prescription will be written and/or refilled.
Dr Chen's office does not e-prescribe, call-in, fax, or mail prescriptions on your behalf. Our office does not complete prior authorization for medications.
Any request for a change in dosage or type of medication not provided for in your care plan requires an in-office or phone appointment.
Prescriptions are written at appointment times only. This includes lost and/or expired prescriptions. If a prescription is lost, a new prescription will be written only at the next refill allowable time.
When a new drug is prescribed, the prescription will be written with no refills. At the follow-up appointment, Dr Chen will determine if changes are necessary before writing for another medication or refill. Once therapeutic success has been demonstrated, at your subsequent follow-up appointment, Dr Chen may authorize refills for extended time period (ie. 3 month or 1 year supply), condition and medication dependent.
Pharmacy laws place time limits on prescriptions. Even if refills remain on the original prescription, depending the drug, prescriptions expire after six months or one year from the date the prescription was issued and written.

CONTROLLED SUBSTANCES
An appointment is required for prescription medications that are considered “Controlled Substances” (ie. select pain medications). These medications cannot be called in and/or sent electronically to a pharmacy. A written prescription from Dr Chen is required. Prescriptions cannot be altered, crossed out or changed. It is a Felony to do so, and as a result, the prescription becomes void. Prescriptions for controlled substances expire 30 days after the date of issuance.

Dr Chen and her staff will notify you whether the appointment can be either an in-person visit or evisit/telehealth visit
To better meet your needs and to ensure a timely response as well for continuity of care for you, please allow at least 1 week notice to complete any required forms, consents or letters. Failure to give proper notice for completion of the form may result in a delay in meeting your request. If immediate service is required and your request can be accommodated, you will be billed at one-and-a-half times the usual standard rate.

EMERGENCIES, URGENT CONCERNS, HOSPITALIZATION
At the present time, Chen Skin & Cancer Surgery, PA, does not provide inpatient care. Should the need for hospitalization arise, the hospital will assign a physician to care for you during your hospitalization. Your provider will assist the hospital physician with coordination of care.
This facility is not designed to respond to emergency situations. If you have an emergency, please call 911 or go to your nearest emergency room.
If you need urgent or emergent medical advice outside of usual business hours, please contact office specifically stating that your medical issue is "an urgent or emergent matter". Note that these out-of-office communications will be billed at one-and-a-half times the usual office visit fees. If you do not receive a response within 2 hours, kindly reach out to the office again as the correspondence may not have transmitted properly due to either technical difficulties or poor internet signal. If the correspondence is deemed non-urgent and non-emergent, the message will be responded to by the end of the next business day. Please reach out to us again if you do not hear from us by the end of the next business day. There are several convenient ways to reach Dr Chen and her staff (see below).

A staff member will get back to you within 1 business day during the workweek, and most likely sooner. A response over weekend or holiday hours may take a little longer.
For reference, non-medical questions include those about scheduling and rescheduling, pharmacies, prescription issues, lab problems, office hours, directions, medical costs, forms, and other similar topics.
Our staff is not able to dispense medical advice, so questions about medication doses, labs to order, why you are not feeling well, advice about referrals, advice about supplements, new medical topics, and feeling acutely ill should be addressed by your provider in your appointment.

INSURANCE
MEDICARE
New patients with Medicare require a referral from their primary dermatologist.
Dr Chen is a provider for Medicare. On the day of your Specialist Consultation, your insurance company requires that our office collect co-pays, deductible, and/or coinsurance. Your insurance company determines the exact amount you will be responsible for based on negotiated rates for specialist consultation visit fee and procedure costs.
It is likely that our office will have underestimated the exact amount you are responsible. In this instance, you will receive notification from our billing department. Thank you in advance for your patience as the process of dealing with insurance companies can take weeks to months to resolve a single insurance claim. If you choose to delay payment, your account will accrue additional fees and interest, as well as, sent to collections with negative consequences on your credit rating.

ALL OTHER COMMERCIAL INSURANCE PLANS
Dr Chen is able to participate in the care patients with any insurance except Medicaid. Except for Medicare, this office has elected to discontinue contracts with all commercial medical insurances and will no longer be considered an in-network physician effective July 1, 2019. After your visit, our office can prepare documents for patients to submit to the insurance to apply to deductible, out-of-pocket max, and for reimbursement if eligible for out-of-network services.
Patients may receive a reimbursement for their visit (ie. for instance, $120 of a $150 out of pocket expense), but this can depend on a multitude of factors including eligibility, remaining deductibles, coinsurance, and/or out-of-pocket maximums. Many of commercial medical insurance plans allow patients to see out of network doctors (such as PPOs from Aetna, Blue Cross Blue Shield, Cigna, Humana, United Health, etc, as well as, Medicare Advantage plans). HELPFUL TIPS TO FILE A CLAIM
The fees for your visit to Chen Skin and Cancer Surgery, PA, may be covered by your insurance plan as an out-of-network benefit. If our office does not contract with your commercial medical insurance companies, we will not file your claims. After you pay for the services at the time of your visit, you will be provided with an itemized bill that has the required codes that you can file with your insurance for possible reimbursement or application to your deductible if you have a high deductible plan.
If you have a huge deductible, keep your receipt. Insurance companies have different policies regarding payments to out-of-network providers. Some allow those payments to count toward the deductible, and some do not. Submit only if you think you may reach your deductible.

- Attach the itemized receipt with the required procedure codes (CPT) and diagnoses codes (ICD) from your visit to Chen Skin and Cancer Surgery, PA.
- Check the form for accuracy.
- Make a copy of the completed claim form and all attachments for your records.
- Mail the completed claim form and attachments to the address indicated on the form for processing of the claim.
- You should write down the date you mail the form, since they will ask this information if you call the insurance company to check on the status of your claim.
Some patients have spoken with their insurance company about "network deficiencies". They have specifically asked their insurance company if there is any physician in proximity of the patients’ residence, who is in network and who specializes in surgical dermatology and skin cancer management, including MOHS micrographic surgery and reconstruction for skin cancer. If no physician can be identified, then some insurance companies may consider this a "network deficiency" and may choose to cover the consultations, procedures, and surgeries and may then reimburse you at in network rates.

HSA, HRA, FSA
You may use these accounts at Chen Skin and Cancer Surgery, PA, for valid medical care. HSAs, HRAs and FSAs are accounts used to save on taxes and pay for qualified medical, prescription, dental and vision expenses. You may need a coded receipt to prove the service was valid medical care. If you buy your own insurance, you do not need to worry about comparing all these accounts. That is because you are only eligible for an HSA. Employers can offer any of these accounts, and sometimes more than one at the same time. Spelled out:
- HSA | a health savings account is a bank account that lets people put money aside, tax-free, to save and pay for health care expenses. The Internal Revenue Service (IRS) limits who can open and put money into an HSA. Due to legal restrictions, HSA balance information cannot be displayed.
- HRA | a health reimbursement account or arrangements are health care accounts that employers fund for covered workers or retired persons. The IRS does not tax this money and allows any money left in these accounts at the end of the year to roll over to be used the next year.
- FSA | a flexible spending account or arrangement is a health care account is a special account you put money into that you use to pay for certain out-of-pocket health care costs. You don’t pay taxes on this money. This means you’ll save an amount equal to the taxes you would have paid on the money you set aside. Employers may make contributions to your FSA, but aren’t required to.

- Checks returned for nonpayment will be charged $75 plus any bank charges incurred as a result.
- Late fee for each month with no payment arrangement on accounts with an outstanding balance will be $75 monthly.
- If services are not paid in full at the time services are rendered, a statement will be mailed at the beginning of the following month detailing account balance.
- Accounts will be sent to collections after failure to settle accounts after 90 days in arrears.
- Record copying fee is mandated by Texas Law and the Texas Medical Board. Contact the office for instructions and fee schedule.
- Completion of forms will be billed at the usual standard rates.
- Letters to insurance companies will be billed at the usual standard rates.
- Coded receipt for patients that wish to submit their claim to insurance for possible reimbursement is $10.
- In-office dispensed skin care products, supplies, and medications: subject to availability.
- External services such as labs, dermatopathology, and pharmacy-filled prescriptions may be available at a special negotiated fee or patients can choose to use their insurance; fees for theses services can range from $150 to $300.
- Patients who no show or cancel/ reschedule within 48 hour of their reserved appointment time will be subject to a Missed Appointment Fee or Forfeit of Deposit.

- Usual standard rate is billed $50 for each 5 minute increment.
- Surgery and procedure add-on fee determined at time of new or established patient appointment. Usual standard rate for procedures and surgeries is billed $75 to $275 for each 5 minute increment. The estimate is based is various factors that can only be determined at time of new or established patient appointment (ie. lesion size, complexity, location, supplies required, anesthesia, procedure time, medications, etc.).
- Physician availability outside-of-regular office hours for appointments, correspondence, and/or handling issues on your behalf will be accommodated upon request and will be billed at one-and-a-half times the usual standard rate.
